This page is provided as an example of how clients give consent to receive SMS text reminders at Andelige Trauma Counseling & Consulting, PLLC.
For HIPAA compliance, all actual consent is collected securely through our client portal (SimplePractice) or via phone during the intake process. The images below are screenshots from our secure portal, showing the SMS consent checkboxes that clients complete.
This page is for demonstration only — no personal information is displayed, and you cannot provide consent here. Actual SMS consent steps as seen in client intake.
Step 1: Intake Documents Screen
Clients receive intake forms in the secure portal. One of these is the Demographics Form, which includes communication preferences.

Step 2: Communication Preferences
Inside the Demographics Form, clients can check a box to say “Yes, it’s okay to send text messages to this number.” This is how SMS opt-in consent is recorded securely.

This screenshot shows our SMS Text Messaging Consent step. Clients must actively check the box to opt in. Consent includes program purpose, frequency, STOP/HELP instructions, disclosure that consent is not a condition of care, and links to our Privacy Policy and Terms.
**Note:** This page is for compliance demonstration purposes only. Actual consent is collected inside our HIPAA-secure portal or by phone with staff assistance.
