In Case Management (CM) sessions you will meet with a case manager trained in assessing your needs. Together you will partner in working to meet those needs through linkage, advocacy, and referral assistance. A case manager can help you navigate the healthcare system and refer you to needed resources in the community. They can support you in making phone calls to other providers and assist in advocating for your needs.
Sessions are typically at least 30 minutes in length and can run longer in 15-minute increments. Multiple family members can be present in one session. Common needs identified and navigated in session include low-income housing, social security disability, access to food and clothing, locating providers such as psychiatrists and other specialists, coordinating care between providers and primary care physician, and much more.
